Yes those people are at fault but not as much as the flight crew are. The pilot in command of a civil airliner has a huge responsibility. Every decision theymake must be made with the safety of their passengers as the first concern.

In particular, there was no excuse for their ignoring the minimums call for 3 minutes, there is no excuse for their attempting a visual approach in those conditions and there is NO excuse for 2 experienced pilots to get the TOGA procedure wrong.

Similiar to the American 965 Disaster in Cali, Colombia, because the captain order his copilot to put on the speed brakes but when they got the GPWS Alarm, they did pull up, but the speed brakes were still deployed so the plane's rate of descent was great, so it impacted the mountain, only 3 survived or so.

On the official transcript of the cockpit voice recorder, 3 whole minutes after the minimums call the FO says "not in sight, missed approach" then after the capitain takes no action he repeats "missed approach".

Why didnt he begin the go around himself? The captain of an aircraft, like the master of a ship, used to be beyond question. It was unheard of for a first officer to tell the captain he was wrong.

For this reason, 80% of controlled flights into terrain happen with the capt flying

fightingtemeraire: i know nothing about airplanes and aviation, but what you say does makes sense. i guess the pilot made bad judgement cuz he was kinda tired...
but why didnt the co captain say anything? he didnt seem tired also...
